- Once you have
organized your tax paperwork, you will be ready to complete
our form(s)*. You may
submit your return to us
after you have completed filling out the necessary forms below. The forms are easy to complete and only
collects
information needed to process your return. Your forms and tax
documents are kept secured
in our office, as stated in our
Privacy Policy.

- *All clients must complete at least one
form - either the New Client or Returning Client form.

- Note: The forms below are Microsoft®
Word documents, thus they can be accessed by using MS Word. To save
the form, right click on its link and choose "Save Target As". In order
to complete the forms using a computer, click on the text boxes before
typing. If completed by hand, please write legibly. You may
need to temporarily turn off your pop-up blocker as well. Please
inform us
of any difficulties. Thank you.

Either a New or Returning Client Form
All clients must
complete either a New or Returning Client form. We
will not process your return if you fail to submit one.

New Client Form |
This form is to be completed by clients who had not
filed their tax returns with us last year. We use
this form to determine your filing status,
dependents and contact information. You may be
prompted to complete additional forms. |

Returning Clients |
Clients who have
filed their tax returns with us last year are our
cherished Returning Clients. We require all
out-of-state clients or those who cannot visit our
office to complete a Returning Client Form.
If you plan on submitting your paperwork in
person, you are no longer required to fill out this
form. However, we
will check in with you to note changes in filing status,
dependents, bank info, addresses and contact information
before we process your return. You
still may be prompted to complete additional forms. |

That You May Need to Complete
Based upon the tax
statements you have received and your how you answered the
final questions in the forms above, you may need to complete
one or more of these additional forms.

Mover's Form |
This form is for qualified tax filers who wish to
claim moving expenses. To qualify, you must
have either entered the U.S. in 2006 or moved 50
miles or further within the U.S. If you moved
within the U.S., you must also prove at least six
months of residency to qualify. You cannot
claim costs that your employer has paid on your
behalf. |

Stock Form |
If you bought and sold stocks in 2006, this form
will guide you on how you should report your stock
transactions. This form isn't only convenient for
us and good for your organization, but the IRS
prefers that you record your stock transactions like
this too! |

Homeowner's Form* |
If you have owned, purchased or refinanced a home in
2006, you must fill out this form. *If you
have purchased or refinanced in 2006, you must also
submit your Closing Statement so that we may claim
your closing costs. |

Education Credit
Form |
If you, your spouse or child attended college or
university in 2006, you must complete this form.
The college or university should have issued a
1098-T, which is needed to complete this form and
claim your education credit. |

ITIN Application

Exclusively for our H1-B clients, we can help you
apply for an IRS Individual Taxpayer Identification
Number (ITIN). An ITIN is a nine-digit number
issued by the IRS so that taxpayers on H1-B visas
can claim their spouses, children, parents, etc. as
dependents, even though they do not have a SSN.
The ITIN is for federal tax purposes only. It
does not entitle you to
social security benefits and does not change your
immigration status or
your right to work in the United States.
